What You Should Know

(and Why Early Enrollment Matters)

Let’s discuss how we can bring comfort & dignity home

Many families assume hospice is only for the last days, but the truth is that earlier referrals can offer months of comfort, symptom management, and meaningful time together. Giving yourself time to adjust and plan can make the entire journey more peaceful and purposeful.

Hospice services are often covered by Medicare, Medicaid, and most private insurance plans. This gives your family one less thing to worry about during an already difficult time.

Our Team is Here for You
Home Health Aides
Our home health team can help create independence and safety in the areas of personal care. Our reliable staff can assist with activities of daily living such as bathing, dressing, toileting, and light meal prep/housekeeping.
Physician Services

Our health care team of professionals work under the direction of your personal physician to improve your health in the comfort of your own home.

Patient Care Volunteers

Our well-trained hospice volunteers make home visits and provide supportive services such as spending time with the patient to allow respite for loved ones, reading to the patient, active listening and just “being there.” Volunteers also help by running errands and going grocery shopping.

Spiritual Support
Our Hospice Chaplain is available to provide spiritual guidance and direction for the patient and loved ones upon request. If the patient and family does not have a religious affiliation or has not been active in a tradition, the Chaplain may be called upon for matters of faith.
Social Services

Hospice social workers help both patients and their families navigate the difficult process of end-of-life planning; manage the mental, emotional, familial, and monetary stressors of debilitating physical illness; understand their treatment plan and be vocal about their needs; overcome crisis situations; and connect to other support services in the area.

Grief Counseling
There are many emotions which are not only difficult to define but are even more challenging to understand. Among the most demanding and confusing to experience is grief. Grief can manifest itself in many ways, but generally is described as a deep sensation of sadness, especially after loss.
